Home Education Information & Required Annual Forms

Grand Forks Public Schools is committed to supporting families who choose to home educate their children. Each year, families participating in home education must complete and submit the necessary forms to ensure compliance with state requirements and to maintain updated records with the district.

Quick Guide for Home Education Enrollment

Below you will find important information and links to the required documents for the upcoming school year. These forms and records must be submitted annually or updated as needed:

  • Statement of Intent (SOI) 
    • On this form please be sure to indicate your testing decision as well as if monitoring is applicable to your family. 
  • Standardized Testing Form
    • If you are planning to utilize the district's testing for your 4, 6, 8, or 10th grade student(s) the form is listed below. 
  • Birth Certificate 
  • Immunization Records 
  • Parent Qualifications - If you do not have one of the following, please indicate that on your statement of intent with the need to have a monitor for your student(s). 
  1. Documentation for parent qualifications can be any of the following; 
  2. High School Transcript - Official with graduation
  3. GED
  4. College Transcript - official or unofficial 
  5. Degree - BA, BA, MA, MS, PhD, etc. 
  • Additional Information 
    • Process of transferring into the schools 
      • Call the District Office (to assemble any necessary documentation and to being powerschool transition)
      • Call your resident school (see what they would require of you)
      • What the school might request of you 
        • Transcripts, testing scores, any documentation in regard to their education, immunization records, Birth Certificate, or more. 
    • Homeschool Handbook 
    • Century Code
    • Curriculum Century Code
    • Activities or School Events 
      • Please call your resident school's Principal to see the events throughout the year that you would be able to attend. Link to find your resident school: https://app.guidek12.com/grandforks/school_search/current/ 
      • You will only be able to attend the events the administration approves for your resident school.
